How to Get Started

Note: If you are planning to use someone you know as a donor, please read about our Directed Donor Program or request a Directed Donor Packet from our office.

Choosing parenthood often involves many complex decisions, so we would like to help simplify the process of getting pregnant for you. If you are unfamiliar with the variety of artificial insemination methods and specimen types, read About Specimen Types.

The following information will get you started:

  1. First, get registered

    We are required by government regulations to obtain your basic contact information and informed consent to ensure that you're aware of medical and legal matters related to using donor sperm. The required forms can be downloaded by choosing forms for a Single Woman or for Couples and mailed, emailed or faxed to us. There is a one-time fee of $150 for the mandatory intake consulting appointment (see number 3, below). Once we receive your registration forms, we will contact you to set up your intake appointment.

  2. Next, request sperm

    You must register in order to place a sperm request. You may submit your registration forms, intake fee and Sperm Request Form together by fax or you may place a sperm request by phone or email. Once you are an established client, you can submit the Sperm Request Form online.

  3. To receive sperm

    • Submit all of the Registrations Forms
    • Complete an Intake Consulting Appointment with our Clinician ($150)
      This is a very important consultation in which we will discuss the different types of reproductive procedures available and help you decide which would be best suited for you. We will also review with you how to detect ovulation, order specimens and schedule sperm transport, if needed. Our donor screening process and "willing to be known" donor program will also be reviewed at this appointment. (For clients living outside the San Francisco or Los Angeles areas, these appointments may be conducted on the telephone).

    • Get Medical Clearance
      Before becoming pregnant, it is important to screen for any pregnancy-related medical risks for you or your child. If you live in the San Francisco Bay Area or the Los Angeles areas, this can be done in a single visit to our office, and you can be ready to inseminate in less than a week (if a pap smear is done, it may take about 2 weeks). Alternately, you can get Medical Clearance from your own clinician (physician, nurse practitioner, nurse midwife or physician's assistant) using our Medical Certification Form, which you or your clinician can return to us.

It is important to understand that when you have placed an order and paid for sperm it cannot be released to you without completion of the Intake Appointment and Medical Certification. Pre-paid sperm orders are held at no additional charge for three months. After three months storage fees will apply.
